Memory catchers, assisting in making video of the Patients life, using a series of lifetime questions. Bringing the conversation back to continue to make a video for the patient and families for a continued legacy of their loved ones lifetime.

Volunteers that see patients 
here are the requirements:
#1 Orientation
#2 Background check
#3 Drug screen
#4 TB test on hire and Annually
#5 Flu shot 
#6 HEP B- or titer  You  must continue to Volunteer during this process- 6 Months
1-6 are paid for by Hospice no out of pocket cost for the Volunteer.

Volunteer must provide on an ongoing basis the following:
A. Valid Drivers License
B. Active Car Insurance
C. Provide Social Security Card for Proof of Social Number accuracy
D. Documentation of visits with patients within 24 hours of visit
E. On going Education thru out the year and Annually.
F. Following Supervisor instructions and communication with Supervisor
G. Following the Plan of Care for the Patient
H. Strict Guidelines of confidentially when discussing Patient information. HIPAA 

All Volunteer Applicants will be interviewed
